MOULTRIE, Ga. -- Hazel Lenora Phillips Lewis, 85, of Moultrie, Ga., died Monday, November 27, 2017 at Colquitt Regional Medical Center.
Graveside rites will be held 2:00PM Wednesday, November 29, 2017 at Old Prospect Cemetery in Fitzgerald, Ga., with Mr. Joey McMurphy officiating.
Born January 10, 1932 in Fitzgerald, Ga., she was the daughter of the late Laura Jane Gray. Mrs. Lewis was formerly employed with Wells Aluminum and retired as a waitress from Variety Vending Company.
In addition to her mother, she was preceded in death by her husband, Herchel Wallace McCollum; daughter, Brenda Gail McCollum; brother, Hubert Phillips.
Survivors include five daughters, Kathy Elaine McMurphy and Ronnie, Mary Aileen McCollum and Bernandino, Laura Jane Lollar, all of Moultrie, Ga., Mary Katherine Taylor of Alabama, Sue Yarborough and Melvin of Norman Park, Ga.; fourteen grandchildren, Chris McMurphy and Missy, Ronnie McMurphy, Joey McMurphy and Tammy, Heather Meads and Shannon, Mandy Dale and Bobby Kastelic, Marjie McCollum and Brandon, Gaylon McCollum, Chad McCollum, Crystal McCollum, Rocky Cisneros and Erika, Adriana Gomez and Eric, Kayla Johnson and Christopher, Kim Carter and Robert, Tracy Kirkland and Justin; forty great-grandchildren; six great-great-grandchildren and other family members.
The family will receive friends from 6:00PM-8:00PM Tuesday, November 28, 2017 at Cobb Funeral Chapel.
